Permission Slip   (Youth Needs one filed out for Each Troop Campout or Event Away from the Church)

Service Project form (Scouts must fill out before Starting Star or Life Rank Service Project)

Adult Resource Survey Form (Adults need to fill out before Son joins Troop)

Scout Resource Form (Adults need to fill out when Son becomes a scout with the Troop)

Transportation Form  (Every family needs to have a current form on record,  When you change cars or trucks new form needs to be filled out)

Rappelling / Cave Trip  Information Sheet  (New 1-24-05)
Sheet must be filled out completely and signed by Parents before any scout can go on a Rappelling / Cave trip. Print form and bring to scout meeting.  New form required for each trip.
